3 players: 15 cards in hand + 9 madnesses to get
4 players: 16 cards in hand + 9 madnesses to get
5 players: 15 cards in hand + 10 madnesses to get

I think, it's balanced (and it also feels balanced). With five players you have to communicate more, but a Leader token for more cards gives you 5 extra cards (and we mostly use our Leader tokens for card draw).
