The answer to this is pretty simple. There’s still a minimum viable size for a print run. They have to estimate whether there would be sufficient demand in the Kickstarter campaign to hit that minimum print size. In fact, they’d probably want it to exceed the grey area where a print run is barely profitable and still not worth the time and effort.

If they aren’t extremely confident they will get enough people ordering it through Kickstarter, it’s not worth the risk. Because if they do offer it, they are committed. They can’t offer the add on and then change their mind because not enough backers added it.

Not really. You indicated they did a limited print run corresponding to the campaign anyway - which means they would have had to have had taken on at least whatever the minimum risk would have been to have done that.

If they had just released stock they had on hand, that would be one thing. However, if they did a new small print run to tie into the recent campaign, they were already taking that risk when they could have instead taken less risk with more accurate counts by waiting and getting orders through the KS campaign.
