The answer to this is pretty simple. There’s still a minimum viable size for a print run. They have to estimate whether there would be sufficient demand in the Kickstarter campaign to hit that minimum print size. In fact, they’d probably want it to exceed the grey area where a print run is barely profitable and still not worth the time and effort.
If they aren’t extremely confident they will get enough people ordering it through Kickstarter, it’s not worth the risk. Because if they do offer it, they are committed. They can’t offer the add on and then change their mind because not enough backers added it.
