If you lose sanity and trigger your 4th threshold while carrying out Cthulhu's stage 1 end of turn effect do you resolve the 4th threshold then continue to the 2nd stage and if it causes enough sanity loss to trigger your 5th threshold do you resolve your 5th threshold and the 3rd stage happens and then triggers 6th threshold?

In my recollection, it is not the case the several stages trigger sanity loss directly. But if this were the case, you could lose sanity several times. The threshold is only for one instance of losing sanity: one dice roll, one case of "lose sanity for XXX".
Loss of sanity is lumped together only for cases like making a roll for fire, you roll once for every fire token on you.

Stage 3 requires loss of 1 sanity at the end of each turn, and Stage 4 requires loss of 2 sanity if a R'lyeh token is supposed to be placed but can't be. Based on mechanics and theme, I would agree each stage should be addressed separately, and so sanity could be lost twice.
