I'm not sure what the ability says in other languages, but in the English version it is clear that you don't need a city card at all. Clinic Planner says "you do not need a City card".
There are several actions/abilities later in the campaign where you need to have a city card but don't need to discard it, and they all describe this by saying that you need to reveal the city card. Clinic Planner does not say that.
The Clinic Planner is also clearly based on the Operations Expert role from original Pandemic, who also does not need a card at all.
